Insurers have controlled provider reimbursement while physician costs soar. Providers are leaving practice because of decreasing reimbursement and rising costs (malpractice insurance and business tax). You can only squeeze providers so much and then you have a decrease in quality and shortages. This is an area I have quite a bit of personal experience in.
